All of the forms can be downloaded for completion online and signed digitally. Alternatively, you may wish to print them and complete them by hand.
Completed forms should be returned to the Crematorium at least 2 working days prior to the date of the service, together with a Certificate for Burial and Cremation, (the green form) and Form Cremation 4 completed by a doctor.
Forms can be returned by:
- email to – papers@cannockcrematorium.co.uk
- post to – Cannock Chase Crematorium, Norton Road, Heath Hayes, Cannock, Staffordshire, WS12 3HJ
- or in person to the Crematorium Office.
If you complete form 1, 2 or 3 we also require a Service Details Form to be completed. This provides us with all the supplementary information we need to tailor the service to meet your client’s requirements.
